Please print out the form, fill in the required information and send it in to the address on the bottom of the form for processing to receive your certificate. It is mandatory to fill out email address on CEU submission forms. This will enable a more accessible communication between Krista and members in regards to any questions members may have. • ONE UNIT
  Cardiology Rounds in your hospital or clinic, per hour
  Cardiology Books, per hour, maximum 5 each book
  Web Reading, per article submitted with brief summary
  Research Day, per hour
  Cardio Videos, per hour
  Inservicing Allied Health Professionals, preparation & presentation, per hour, maximum 5
  
• THREE UNITS
  A.S.R.C.T. Committee, per year
  
• FIVE UNITS
  A.S.R.C.T. AGM
  A.S.R.C.T. Executive Office, per two year term
  
• FOUR TO EIGHT UNITS
  CPR, depending on length of course
  
• EIGHT UNITS
  Prepare and Present a talk related to Cardiology (1 hour)
  
• TEN UNITS
  Canadian Cardiovascular Conference (full conference)
  
• FIFTEEN UNITS
  Per Cardiology Technology course from a C.S.C.T. approved school
  Preceptor, for every student trained
• OTHERS
  Other Cardiology related courses, 1-8 Units depending on length
  Conduct and publish a Research report, 30 Units primary author, 25 Units co-author
